Społeczność, egzystująca na terenie rosyjskiego metra, z pewnością nie ma łatwego życia. Nie brakuje co prawda czystej wody oraz tlenu (dzięki skonstruowanym systemom oczyszczania), elektryczności (generatory prądotwórcze), a nawet żywności produkowanej w podziemnych farmach, ale wszystko to nie zmienia jednak faktu, iż epoka dominacji człowieka nad Ziemią odeszła do historii. Żyjące na powierzchni zwierzęta uległy bowiem różnego rodzaju mutacjom, stając się bardzo groźnymi monstrami. To właśnie te organizmy z powierzchni pewnego razu zaczęły nękać mieszkańców północnej strefy schronu, zwanej VDNH. Z czasem ich ataki stały się coraz groźniejsze, a zwarta defensywa, zorganizowana przez mieszkańców tej części obiektu, z czasem została silnie przetrzebiona. Gracz rozpoczyna rozgrywkę od przejęcia kontroli nad jednym z młodych mieszkańców VDNH, który ma za zadanie przedostać się do legendarnego Polis, centralnej części metra, powiadomić tamtejszych mieszkańców o zbliżającym się niebezpieczeństwie i jednocześnie poprosić o wsparcie nadwątlonych linii obrony. Oczywiście podróż do serca schronu nie jest usłana fiołkami, nie wspominając już o różach – na każdym kroku czyhają bowiem niebezpieczeństwa, gwarantujące potencjalnemu graczowi nieprzerwanie szybką akcję w podziemnych tunelach moskiewskich.
